Bilingual Community Health Navigator
Bronx Community Health Network
Bronx Community Health Network (BCHN) is hiring a Community Health Worker (CHW) to join the Population Health team. The CHW will promote health, reduce barriers to care, and support patients in navigating the healthcare system. This is a full-time, on-site role with a Monday–Friday schedule and some weekends as needed, reporting to the Asst. Director of Population Health. The CHW will conduct outreach, assist with appointments, provide education, and document interactions in the EHR while #J-18808-Ljbffr Bronx Community Health Network
