Work Environment

This role is primarily onsite, meaning you will travel to patients' homes to administer care. Key Responsibilities

• Deliver high-quality, patient-centered care by overseeing the complete process of therapeutic interventions for an assigned group of patients.
• Perform thorough patient assessments to identify needs and develop individualized care plans focused on achieving rehabilitation goals, enhancing patient engagement, and ensuring a positive therapeutic experience.
• Carry out evaluations and treatment interventions, ensuring compliance with established laws and ethical guidelines.
• Utilize evidence-based methods to address therapeutic, rehabilitative, and functional issues, participating actively in formulating overall care strategies.
• Conduct physical therapy sessions at the patient's residence, applying specialized techniques and tools in alignment with the physician's treatment plan.
• Educate patients, their families, and caregivers about therapeutic exercises, appliance usage, and home care techniques, empowering them to continue rehabilitation outside of scheduled visits.
• Manage your caseload effectively, adjusting priorities as needed and ensuring appropriate patient hand-offs among care teams.

Patient Population / Client Focus

Our clientele includes individuals recovering from surgery, older adults managing chronic conditions, and pediatric patients with developmental challenges. You will engage with patients of varying ages and backgrounds, adapting your approach to meet their unique rehabilitation needs. Your role as a Physical Therapist is to not only restore physical function but also to instill confidence and encourage active participation in their recovery journey.

Qualifications

• A valid license to practice as a Physical Therapist in the state of California.
• A Doctor of Physical Therapy (DPT) degree from an accredited institution.
• Basic Cardiac Life Support (BCLS) certification is required within the first 30 days of employment.
• While no prior experience is necessary, a passion for patient care and strong interpersonal skills are essential.

Preferred Experience

• Previous experience in a home health setting or community health environment is desirable but not a strict requirement.
• Familiarity with therapeutic modalities and home care equipment.
• Ability to work independently while effectively collaborating with interdisciplinary teams.

Compensation and Benefits

We offer an attractive salary range of USD 100,000 – 120,000 per year, commensurate with experience and qualifications. Our comprehensive benefits package includes:
