Position Overview

The Health Coast Behavioral Center is seeking a dedicated Licensed Physical Health Therapist to join our team in Ontario, California. This full-time position involves working directly with patients in a home care setting, focusing on improving physical health outcomes through evidence-based therapy techniques and personalized treatment plans. Work Environment

As a physical health therapist, you will work onsite in the patients' homes, immersing yourself in a dynamic, hands-on environment where personalized care is paramount. This role provides the unique advantage of building meaningful relationships with patients, allowing for direct observation of their environments and tailored intervention strategies that better facilitate their rehabilitation process. The usual work schedule is Monday to Friday, 9:00 AM to 5:00 PM, with some flexibility required based on patient needs.

Key Responsibilities

• Conduct comprehensive assessments of patients' physical health status, designing individualized treatment plans that address their specific rehabilitation needs.
• Implement evidence-based therapeutic interventions, guided by professional best practices, to enhance patients' physical mobility and overall health.
• Educate patients and their families about therapeutic techniques, health maintenance, and injury prevention to encourage active participation in their recovery.
• Collaborate closely with multidisciplinary staff, including psychiatrists, social workers, and other healthcare providers, to ensure holistic treatment approaches for each patient.
• Document patient progress meticulously, maintaining compliance with legal and ethical standards, to ensure continuity of care and effectiveness of interventions.
• Provide crisis intervention services when necessary, helping patients manage acute physical issues that may arise in the home setting.
• Stay current with advancements in physical therapy practices and contribute to the continuous improvement of treatment protocols within the organization.

Patient Population / Client Focus

The patients you will support range from children to older adults, many facing chronic conditions, post-operative rehabilitation needs, and disabilities that impact their physical health and quality of life. Your expertise will be crucial in promoting independence and facilitating better physical functioning among individuals dealing with varying degrees of mobility issues, adapting your methods to address diverse backgrounds and challenges.

Qualifications

• Must possess a Bachelor's Degree in Physical Therapy from an accredited institution.
• Current licensure as a Physical Therapist in the state of California is required.
• Demonstrated knowledge of physical therapy best practices, rehabilitation concepts, and effective treatment modalities.
• Exceptional interpersonal skills with a focus on patient-centered care and empathetic communication.
• Strong problem-solving abilities and a commitment to ethical practice standards.

Preferred Experience

• Previous experience in a home care or community health setting is preferred.
• Experience using electronic health record (EHR) systems and familiarity with clinical documentation practices.
