Work Environment

Located in Rancho Cucamonga, California, Heal Motion Care operates primarily in-home care settings, allowing our team of therapists to provide convenient and effective therapy services in the comfort of clients' homes. This full-time position allows for flexible scheduling Monday through Friday from 8 PM to 3 PM, enabling therapists to optimize their work-life balance while remaining responsive to client needs. Key Responsibilities

• Conduct thorough Physical Therapy evaluations for children and adults, assessing mobility, strength, balance, and functional capabilities.
• Develop and implement individualized treatment plans designed to address specific needs, utilizing evidence-based techniques and therapeutic modalities.
• Continuously monitor and assess clinical progress, adjusting treatment strategies and goals based on client response and recovery trajectories.
• Maintain accurate and detailed documentation of patient progress, including SOAP notes and care plans, using Heal Motion Care's electronic record system to ensure timely communication and record-keeping.
• Educate clients and their families on recovery processes, setting realistic expectations, and addressing potential challenges throughout treatment.
• Meet established productivity and quality goals as part of business objectives while maintaining a focus on patient-centered care.
• Collaborate with a multidisciplinary team, including occupational and speech therapists, to provide comprehensive services for clients requiring multiple therapies.
• Advocate for and embody Heal Motion Care's core values as a representative and ambassador of our company culture.

Patient Population / Client Focus

As a Physical Therapist at Heal Motion Care, you will work with a diverse range of patients, including children with developmental delays, adults recovering from surgery or injury, and elderly clients seeking mobility assistance. Your role is not only to treat but to empower individuals from various backgrounds, adapting therapies to meet their cultural and personal needs. Qualifications

• A Doctor of Physical Therapy (DPT) or a Master's degree in Physical Therapy from an accredited program is required.
• Current and unrestricted licensure from the state of California's Board of Physical Therapy.
• Willingness to conduct evaluations and treatment both in-person and via telehealth, ensuring continuity of care for all patients.
• A strong desire to engage with a wide variety of patients and a commitment to serving diverse populations with empathy and respect.

Compensation and Benefits
