Key Responsibilities

• Deliver exemplary patient-centered therapy for a designated group of patients, coordinating closely with physicians and the broader care team.
• Conduct thorough patient assessments to inform the development and implementation of individualized therapy plans tailored to each patient's goals and progress.
• Manage and oversee the delivery of therapy sessions, ensuring consistency and alignment with treatment plans throughout the patient's rehabilitation journey.
• Provide evaluations and interventions in accordance with clinical guidelines and ethical standards to maximize functional outcomes.
• Document patient progress meticulously and adjust care plans as indicated to meet the evolving needs of each patient.
• Administer physical therapy within patients' homes, utilizing effective techniques to promote rehabilitation and recovery as per physician directives.
• Teach patients, families, and caregivers the appropriate therapeutic exercises, appliance use, and selves-management strategies to support ongoing recovery outside therapy sessions.
• Manage a caseload efficiently, making daily staffing decisions that optimize patient care delivery and team performance.

Patient Population / Client Focus

Your primary focus will be on orthopedic patients recovering from surgeries, injuries, or chronic conditions. This diverse group includes individuals of all ages—both adults and children—who require specialized therapy to regain mobility and enhance their quality of life.

Qualifications

• Must possess a valid Physical Therapy License in the state of California.
• A Doctor of Physical Therapy (DPT) degree is required.
• Basic Cardiac Life Support (BCLS) certification is necessary; candidates must be able to obtain this certification within 30 days of employment.
• Recent graduates are encouraged to apply; no prior experience is required.

Preferred Experience

While not mandatory, experience in an orthopedic setting or familiarity with home care rehabilitation models will be beneficial. Candidates with any specialized training in patient engagement or advanced therapeutic techniques may have an advantage.

Compensation and Benefits
