Heal Motion Care is seeking dedicated and skilled Physical Therapists to join our team in Fontana, California. As a full-time member of our rehabilitation facility, you will play a vital role in empowering patients to enhance their physical well-being and overall quality of life. Work Environment

The ideal candidate will work onsite at our modern rehabilitation facility in Fontana, California, where you will collaborate with a dynamic team of healthcare professionals. Our facility is equipped with state-of-the-art rehabilitation tools and technologies to support effective therapy interventions. You will work a structured schedule from Monday to Friday, from 8 AM to 3 PM, providing stability and convenience to both staff and patients.

Key Responsibilities

• Conduct thorough evaluations of patients through detailed assessments of their physical abilities and rehabilitation needs.
• Develop, implement, and adjust personalized treatment plans that aim to improve functional mobility and reduce pain, based on individual evaluation outcomes.
• Regularly monitor and re-evaluate patients during subsequent therapy sessions to assess improvements and modify treatment plans as necessary.
• Complete meticulous documentation, including SOAP notes and progress summaries, using our electronic record system to track patient's progress and compliance timely.
• Provide education and resources to patients and families regarding postoperative care, recovery processes, and rehabilitation challenges to promote transparency and involvement in their healing journey.
• Work towards achieving minimum utilization goals and participate actively in fulfilling wider operational objectives of the rehabilitation facility.
• Collaborate effectively with peers across disciplines to ensure comprehensive care for clients receiving multiple therapy services.
• Actively promote and embody the core values of Heal Motion Care in all professional interactions and activities.

Patient Population / Client Focus

You will engage with a diverse patient demographic, including children and adults experiencing various physical challenges or recovering from injuries. This role requires a holistic approach to patient care, addressing not only their physical needs but also their emotional and psychological well-being as part of the recovery process.

Qualifications

• Must possess a Master's or Doctorate in Physical Therapy from an accredited program.
• Current and unrestricted licensure in California, by the Board of Physical Therapy.
• Willingness to conduct evaluations and treatments both in-person and through telehealth methods, when clinically appropriate.
• A strong commitment to serving a diverse patient population, adaptable to various cultural backgrounds and needs.
• Ability to maintain a flexible attitude and rapid response to changes in a dynamic work environment.
• An enthusiastic team-oriented approach, with a strong focus on customer service and patient satisfaction.

Preferred Experience

• Experience working with a range of conditions such as neurological disorders, orthopedic injuries, and pediatric populations.
• Familiarity with electronic health record (EHR) systems enhances efficiency and supports patient care.
• Engagement in professional development initiatives to keep abreast of the latest in therapy practices and advancements.

Compensation and Benefits

The compensation for this position ranges from USD 100,000 to 120,000 per year, complemented by a robust benefits package including:
