In this full-time role, you will deliver exceptional onsite care to clients in the comfort of their own homes, guiding them through their rehabilitation journey. Work Environment

As a Home Care Physical Therapist with Heal Motion Care, you will work directly in clients' residences in Riverside and surrounding areas. This setting allows for a unique and personal connection with your patients, fostering trust and improving outcomes. You will primarily work between the hours of 8 AM to 3 PM, Monday through Friday, providing flexible and individualized sessions that best meet the needs of each client.

Key Responsibilities

• Conduct thorough evaluations of physical health and function for children and adults, utilizing evidence-based assessments and tools.
• Develop, implement, and monitor tailored treatment plans that reflect the goals and needs of your clients, adjusting these plans as necessary based on clinical evaluations and patient feedback.
• Regularly reassess symptoms and progress towards rehabilitation goals and document findings meticulously using our electronic record-keeping system, including SOAP notes and care plans.
• Consult and educate patients and their families on recovery expectations, treatment options, and steps to manage challenges during their rehabilitation process.
• Meet performance targets related to patient utilization and overall quality of care as outlined by the company.
• Collaborate effectively with a multidisciplinary team to ensure comprehensive care for clients receiving multiple services.

Patient Population / Client Focus

You will work with a diverse range of patients, including individuals with various physical ailments, injuries, or disabilities across different age groups and backgrounds. This multifaceted approach requires not only clinical expertise but also an understanding of the unique challenges faced by each demographic, ensuring that care is both effective and culturally sensitive.

Qualifications

• A Bachelor's degree in Physical Therapy, with a Master's degree or Doctor of Physical Therapy (DPT) preferred, from an accredited program.
• A current and unrestricted state licensure from the Board of Physical Therapy to practice in California.
• Willingness to adapt to varied clinical needs, providing both in-person and telehealth options as appropriate for patient circumstances.
• A strong commitment to serving a broad demographic of clients with sensitivity to diversity in age, race, and ethnicity.
• Excellent interpersonal skills with the ability to work collaboratively in a team-oriented environment focused on patient-centered care.

Preferred Experience

• Experience in home health settings or outpatient therapy.
• Knowledge of physical therapy modalities and evidence-based practice approaches.

Compensation and Benefits
