Key Responsibilities

• Conduct thorough assessments and evaluations to identify patients' physical health needs and establish individualized treatment plans.
• Deliver tailored physical therapy sessions, incorporating therapeutic exercises, manual therapy, and other modalities to facilitate patient recovery.
• Monitor and document patients' progress, adjusting treatment plans as necessary to ensure optimal outcomes.
• Collaborate with physicians, nurses, and other healthcare professionals to coordinate holistic care for patients, discussing updates and strategies in interdisciplinary team meetings.
• Provide education to patients and their families regarding treatment plans, exercises, and self-care techniques to enhance recovery.
• Participate in community wellness events and educational initiatives, promoting physical health awareness and available resources.

Patient Population / Client Focus

You will work with a diverse group of patients, including those recovering from surgeries, injuries, and chronic pain conditions. Our goal is to provide individualized care that addresses varying levels of physical health challenges, while supporting each patient's unique journey to recovery.

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited program.
• Active state licensure as a Physical Therapist in California.
• Strong knowledge of rehabilitative practices and techniques, coupled with a commitment to evidence-based care.
• Excellent communication skills for effective interaction with patients, families, and interdisciplinary team members.
• Proficiency in maintaining accurate documentation compliant with legal and ethical standards.
