Geriatrics Medical Director — PACE Center Leader

Habitat Health in Chicago seeks a Medical Director to lead clinical quality, safety, and participant experience at the PACE center. You will balance direct patient care with team development, program leadership, and fiscal stewardship in alignment with our mission. The role requires an MD/DO, active licensure, 3+ years in geriatrics or related care, and proven ability to lead interdisciplinary teams within value-based care models.
