The LHL Medical Case Manager Team Leader provides leadership, supervision, and support to direct reports while also providing medical case management services to people living with HIV at Erie Family Health Centers.At Erie, we are proud to provide competitive salaries, high-quality health care plans, generous time off benefits, retirement benefits, and more! Any additional skills, experience, and education will be reflected in the compensation offered.Main Duties & ResponsibilitiesTeam Leader Supervision:Provide 1-on-1 supervision, leadership, coaching, and support to direct reports.Complete new HIV intakes, manage the HIV pool in the EMR system, and assign cases/patients to appropriate team members.Manage staff schedules, time-off requests, staffing coverage, and assist with hiring, onboarding, and training.Monitor team workflows, performance metrics, and daily activities to promote efficiency and achievement of program goals.Conduct quality assurance of case management documentation and provide feedback, guidance, and support to direct reports.Communicate new initiatives, workflows, and program changes, provide regular updates to the LHL Program Manager, and assist with special projects and assigned meetings.Case Management and Program Support:Manage a reduced caseload of higher-need, high-complexity HIV+ clients and provide medical and non-medical case management services in accordance with agency policies and procedures.Coordinate medical treatments, treatment adherence counseling, benefits enrollment, resource referrals, and access to Emergency Financial Assistance (EFA) to support positive health outcomes.Refer and link clients to appropriate services, monitor follow-through, and assess medical acuity, including home and hospital visits as necessary.Collaborate with HIV Team members, medical providers, and community agencies; attend team meetings, case conferences, outreach activities, and support groups.Document all client care communication in the electronic medical record and state database and participate in quality improvement projects and program initiatives.Assist with LHL events, provide updates to the LHL Program Manager, attend required trainings and meetings, and perform other duties as assigned.Skills and KnowledgeCPR Certification requiredFamiliarity with issues affecting individuals living with HIV.Verbal and written communication skills.Intermediate proficiency in Microsoft Office preferred.Bilingual fluency (English/Spanish) a plus. 50% of LHL patients prefer to speak in Spanish.Experience2 years of experience in community health or with a similar patient population preferred, plus 1-2yrs of supervisorial experience preferred.EducationMust meet one or more of the following minimum education and/or experience requirements:BA or BS degree in social work, social science, human service, health education or related fields preferred.
