Key Responsibilities\n

• Conduct comprehensive assessments and develop individualized treatment plans to address the unique needs of each client, including the formulation of crisis and pre-crisis management strategies.

• Complete all required documentation promptly, ensuring compliance with regulatory standards and agency policies.

• Utilize evidence-based screening tools in therapy sessions, collaborating with the clinical team to continuously improve service delivery.

• Work alongside various teams to design and implement positive behavior support plans, enhancing client outcomes through tailored interventions.

• Engage actively in all licensing and accreditation processes to maintain the highest standards of practice.

• Act as a liaison to community partners, state agencies, and service systems to broaden resources and support networks for clients.

• Establish effective communication channels to meet contract standards and foster agency mission consciousness within the community.

• Forge and maintain relationships with local, state, and federal agencies to promote programs and seek new partnerships and funding sources.

• Pursue continuous professional development through attending workshops, conferences, and training sessions, contributing to your expertise and representing our agency at various forums.

• Uphold and promote the mission, values, and philosophy of Heredia Counseling Center in all professional interactions.

• Perform additional duties as assigned by the Clinic Director, adapting to the evolving needs of the agency.
