ASG Home Care provides in-home and community-based support services to adults with intellectual and developmental disabilities across New Jersey through the NJ DDD system.

When a Support Coordinator sends us a referral, our job is to make it easy: fast response, the right DSP match, and reliable service that makes everyone involved look good. Position Overview

This is not a back-office admin job. It’s a hands-on coordination and growth role.

As ASG’s Care Coordinator, you’ll own two things: keeping our active cases running smoothly, and expanding the referral relationships that fuel our growth. You’ll be the person Support Coordinators trust to get placements done and the person families rely on when they need something handled.

Key Responsibilities

Care Coordination

• Manage active cases: coordinate DSP staffing, schedules, and coverage

• Serve as the primary point of contact for families and Support Coordinators on active cases

• Handle intake and onboarding for new referrals from first call through service start

• Partner with our operations and billing teams to ensure smooth service delivery

• Track compliance, documentation, and service quality across your caseload

Referral Growth & SC Relationships

• Build and maintain relationships with Support Coordinators and SC agencies across New Jersey

• Respond to new referrals quickly and professionally — speed and follow-through are how we win

• Keep referral partners informed and confident, including when a referral can’t be filled

• Identify new referral opportunities and underserved regions

• Track referral activity and outreach efforts

Qualifications

• Experience in home care, IDD services, social services, case management, or care coordination (NJ DDD experience strongly preferred)

• Existing relationships with NJ Support Coordinators or SC agencies a major plus

• Excellent communicator — comfortable on the phone with families, staff, and agency partners

• Highly organized with strong follow-up skills; able to juggle multiple cases without dropping details

• Self-directed and dependable — takes ownership and solves problems, not just reports them

• Valid driver’s license

Compensation

Competitive hourly base rate/salary plus performance incentives tied directly to referral growth and caseload success

Location

In-office, Jackson/Lakewood, NJ, with field flexibility for SC meetings and community events as needed
