Key Responsibilities\n

• Conduct comprehensive evaluations and assessments of clients to identify their psychological, emotional, and social needs.

• Collaborate with medical professionals and support staff to develop and implement Individualized Family Service Plans (IFSP) tailored to each client.

• Monitor client progress and outcomes continuously, adjusting treatment plans as necessary based on individual family needs.

• Provide education and support to families, empowering them with tools and strategies to enhance their loved one's wellbeing.

• Facilitate communication between families and interdisciplinary teams to ensure a unified approach to care.

• Utilize evidence-based practices and adhere to the Natural Learning Environment Practices when developing intervention strategies.

• Administer evaluations and assessments systematically to align with both Federal and State regulations.

• Build trusting relationships with children, families, and team members to foster a nurturing environment.

• Attend regular team meetings to engage in case discussions and share insights on best practices.

• Stay current on community resources to effectively guide families to additional support services.

Patient Population / Client Focus\n

This position focuses on supporting families with young children who are experiencing disabilities or developmental delays. Our team works closely with pediatric clients, facilitating early intervention strategies that promote physical, cognitive, social/emotional, and communication skills during the critical early years of development. The LCSW plays a vital role in guiding families through the complexities of care, helping them navigate the available services within our hospice setting.

Qualifications\n
