Key Responsibilities\n

• Act as a liaison between clients, families, and outpatient providers to ensure effective communication and continuity of care.

• Engage with insurance companies to navigate pre-certification and utilization management processes, ensuring clients receive the appropriate services.

• Participate in discharge planning and aftercare coordination to facilitate smooth transitions for clients returning to community settings.
\n
• Provide therapeutic meal support to clients, modeling and encouraging healthy relationships with food and nutrition.
\n
• Deliver empathetic, secure, and affirming care that respects the diverse identities and unique circumstances of each client.

Patient Population / Client Focus\n

You will primarily work with older adults and individuals facing various life challenges, including mental health disorders, trauma, and social needs. Our holistic approach addresses both individual and intersecting identities, enabling our clients to feel understood and supported throughout their recovery journey.

Qualifications\n

• A Master's degree in Social Work from an accredited institution.

• A valid LICSW license in the state of Washington is required to practice.

Compensation and Benefits\n
