In-Home Health - Nurse Practitioner or Physician Assistant (Full Time) - Greeley, CO

Job Highlights In‑home health evaluations lasting approximately 45 minutes, including medication and medical history review, physical assessment, and optional diagnostic tests such as diabetic eye exams or spirometry. Use of an iPad and an intuitive clinical workflow for each evaluation; no prescribing, lab ordering or treatment changes required. Responsibilities Engage with patients in the comfort of their own homes to build trusted relationships and help improve health outcomes. Contribute to nationwide efforts to visit millions of people, connecting them to the right care for their unique needs. Travel locally and, when licensed, across states as needed to provide in‑home services. Conduct virtual visits when necessary. Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products such as spirometry and diabetic eye exams. Operate within diverse home environments, managing potential allergens, pets or secondhand smoke while maintaining patient safety. Qualifications Active, unrestricted NP or PA license(s) in the coverage area(s). Board certification (required); multi‑state licenses (preferred). Ability to cover the primary locations of Greely and surrounding area. Pay Range The typical pay range for this role is: $87,035.00 – $187,460.00. The actual base salary will depend on experience, education, geography and other factors.
