As a Home Health Registered Nurse, you'll provide personalized, one-on-one skilled nursing care that helps patients recover from illness, manage chronic conditions, prevent unnecessary hospitalizations, and maintain their independence. You'll build meaningful relationships while empowering patients and families with the knowledge and confidence they need to succeed. No crowded hospital floors. No rushed patient assignments. What You'll Do Perform comprehensive nursing assessments and skilled visits Complete Start of Care (SOC), Resumption of Care (ROC), Recertification, and Discharge assessments Develop and implement individualized plans of care Provide skilled nursing services, including medication management, wound care, disease management, IV therapy as appropriate, and patient education Monitor patient progress and communicate changes with physicians and the interdisciplinary care team Coordinate care with therapists, social workers, and home health aides Complete timely and accurate clinical documentation using WellSky Educate patients and caregivers to promote safety, independence, and improved health outcomes Qualifications Active Maryland Registered Nurse (RN) License Required Valid Driver's License and Reliable Transportation Strong Clinical Assessment, Critical Thinking, and Documentation Skills Home Health Experience Preferred but Not Required OASIS Experience Preferred Experience with WellSky Home Health is a plus Passion for Delivering Exceptional Patient-Centered Care Why Human Touch Home Health?
