This position operates within a remote framework, allowing for flexibility in your daily schedule while working closely with our hospice center team. As an LCSW, you will take on a range of responsibilities integral to patient care, including:

• Conducting thorough clinical assessments to develop personalized treatment plans that adhere to best practices.
• Implementing various therapeutic interventions such as cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT) tailored to the unique needs of each patient.
• Maintaining meticulous clinical documentation in accordance with HIPAA regulations to ensure confidentiality and compliance.
• Facilitating cross-disciplinary collaboration with healthcare professionals to design coordinated treatment strategies.
• Engaging in regular case discussions to enhance your practice and address complex client scenarios.
• Offering educational resources and support to patients and families regarding mental health challenges and community services available.
• Staying informed of current research and innovations in mental health treatment to integrate into your practice.

Your role will primarily focus on providing care to individuals facing significant health challenges, particularly in a hospice setting. You will work with patients who are navigating end-of-life processes, addressing their emotional, psychological, and social needs. Families will also be part of the therapeutic process, receiving support and guidance as they face the various challenges associated with caregiving and grief.

The ideal candidate will meet the following qualifications:

• A Master’s degree in Social Work from an accredited institution.
• A valid license to practice as a Licensed Clinical Social Worker (LCSW) in the state of Georgia.
• A minimum of three years of experience in a clinical environment focusing on psychotherapy services.
• Proficient knowledge of therapeutic interventions and solution-focused techniques.
• Strong communication, interpersonal, and problem-solving skills.
• Ability to work collaboratively in a team environment and manage caseload responsibilities independently.

Experience working in hospice or palliative care settings is highly desirable. Familiarity with the psychological aspects surrounding grief and loss will greatly enhance your ability to provide compassionate and effective support to patients and families during their time of need.
