Description

• Provide education to patients and their families about therapeutic objectives, discharge processes, and home exercise programs
• Maintain documentations of evaluations, treatments, and patient progress in line with facility and regulatory standards
• Participate in care team discussions and interdisciplinary meetings to ensure cohesive patient care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent advanced degree obtained from an accredited institution

Certifications

• Active state licensure as a Physical Therapist
