Key Responsibilities\n

• Conduct thorough psychosocial assessments for patients and their families to identify emotional, social, and environmental strengths and challenges related to their medical conditions.

• Develop, implement, and monitor treatment plans utilizing a variety of social work interventions, ensuring each plan is tailored to the individual needs of patients.

• Collaborate closely with a multidisciplinary healthcare team to formulate discharge plans that respect patients' preferences and needs, and involve family members in the care process.

• Deliver education and counseling to patients and their families regarding adaptation strategies to cope effectively with illness and end-of-life concerns.

• Utilize crisis intervention and provide individual, group, and family therapies as required to address immediate emotional needs.

• Function as a resource for other staff members, offering guidance on psychosocial aspects of patient care and facilitating professional education on related topics.

• Clinically evaluate oncology patients for signs of depression and anxiety, providing short-term counseling interventions as needed.

• Participate in an on-call rotation to respond to after-hour needs and facilitate timely intervention in crisis situations.

• Present at educational conferences, contributing expertise in psychosocial oncology practice to clinicians and stakeholders.

• Engage with the community through educational outreach, consultations, and volunteer opportunities to promote and strengthen local support services for cancer survivors.

The LCSW will primarily work with patients experiencing terminal illnesses, particularly those in hospice care.
