Key Responsibilities\n

• Conduct comprehensive assessments of patients to evaluate their mental, emotional, and social needs, analyzing information gathered from family members and medical data to create a diverse picture of patient health.

• Develop, implement, and periodically re-evaluate individualized care plans that align with patient’s goals, adjusting interventions based on progress and changing circumstances.

• Deliver psychoeducation to patients and their families to empower them with tools and knowledge, enhancing their coping strategies during end-of-life transitions.

• Engage with the latest research and interventions in mental health to continually refine and improve therapeutic techniques, ensuring best practices in care delivery.

• Document all interactions and care provided in compliance with regulatory standards and organizational policies, utilizing electronic health record systems to maintain accurate and up-to-date records.

Patient Population / Client Focus\n

You will be working primarily with patients receiving hospice care, facing complex emotional and psychological challenges associated with terminal illnesses. This vulnerable population requires a high level of sensitivity, empathy, and nuanced understanding of grief and loss. You will also provide support to families who may be facing stress, confusion, and sorrow as they navigate this difficult time.

Qualifications\n

• A Master's Degree in Social Work from an accredited institution.
