Diabetes Educator (RN), Full Time - Days, Endocrinology, Morristown

Principal Accountabilities: Coordinates individualized patient and support system assessment, education, planning and implementation of care goals. Collaborates with the referring providers and interdisciplinary team to work toward optimal patient education outcomes and glycemic control. Maintains professional education standards required to be a CDCES in an ADA approved Diabetes Self- Management Education program. Plans and conducts nutritional programs, under supervision, to assist in the promotion of health and control of disease. Assesses nutritional needs and considers patients' cultural, educational, and learning issues. Provides individualized nutrition education to patients, their families, and/or significant others based on their needs assessment. Serves as a resource on nutrition and dietetics to nurses, health educators or other health care providers in the facility. Communicates patient information to referring providers and appropriate staff in a timely manner. Documents nutrition information discussed, written materials provided, and evaluation of patient's receptiveness and comprehension of the education provided. Performs other related duties as assigned. 11.
