Key Responsibilities\n

• Conduct thorough assessments of clients to identify their specific mental health needs and tailor treatment plans accordingly.

• Maintain ongoing communication among supervisors, direct care staff, families, and other stakeholders regarding client progress and care coordination.

• Develop and share a structured schedule for appointments, home visits, and monitoring activities to keep clients engaged and supported.

• Organize and facilitate home visits for in-depth assessments, including chart reviews and interviews with clients and their support systems.

• Ensure that all aspects of the individualized program plans are executed effectively, adapting strategies as needed to enhance client outcomes.

• Actively participate in Interdisciplinary Team Meetings, fostering a collaborative approach to client care and sharing insights for best practices.

• Document client progress through monthly and quarterly notes to track advancements and inform necessary adjustments in treatment.

• Coordinate training sessions for team members, ensuring proficiency in implementing program plans and maintaining high documentation standards.

• Provide consistent follow-up with clients, engaging in home visits and observations to support their ongoing mental health journey.

• Serve as a liaison to the mental health community, advocating for clients’ needs and collaborating with local organizations to enhance service access.

Patient Population / Client Focus\n

Your work will primarily involve supporting a diverse clientele, including both adults and youth struggling with various mental health challenges. As a Licensed Professional Counselor, you will play a crucial role in delivering sensitive and effective care, primarily focusing on therapy for individuals dealing with trauma, substance abuse, and familial issues.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a closely related field.

• Possession of active and unrestricted licensure as a Professional Counselor (LPC) in the state of Texas.
