Sigma Systems Inc is currently seeking a Community Health Worker / Customer Service Representative to join our client's team. This is an excellent opportunity for a healthcare professional with Medical Assistant and call center/customer service experience who is passionate about member outreach, healthcare education, care coordination, and helping individuals overcome barriers to accessing healthcare services. The ideal candidate will be comfortable conducting face-to-face community outreach, working with Medicaid populations, coordinating with healthcare and community organizations, and providing compassionate support to members.

Job Overview

The Community Health Worker will focus on member outreach, preventive healthcare education, care coordination, and community engagement. The role involves initiating face-to-face outreach with targeted members, including individuals requiring preventive health services, maternity support, care transitions, and chronic-condition assistance.

The candidate will educate members, identify healthcare and social barriers, assist with Health Risk Assessments and appointment scheduling, connect members with appropriate resources, and collaborate with community organizations and healthcare providers. This position requires significant field travel and direct interaction with members and community partners.

Key Responsibilities

Member Outreach: Initiate face-to-face outreach efforts with targeted members related to preventive health, maternity, care transitions, and chronic-condition initiatives.

Health Education: Educate members about the importance of preventive healthcare services and available healthcare benefits and resources.

Care Management Referrals: Assess members' needs and identify opportunities for care management intervention, including referrals to Behavioral Health Managed Care Organizations when appropriate.

Care Coordination: Coordinate with members, healthcare providers, and community agencies to help members access appropriate preventive health services.

Member Engagement: Develop positive, supportive relationships with members and community centers to encourage members to take an active role in their health and wellness.

Health Risk Assessments: Assist members with completing Health Risk Assessments and identify barriers that may affect their ability to obtain healthcare.

Barrier Resolution: Help members address healthcare barriers such as transportation, financial challenges, childcare, lack of healthcare knowledge, and other social or personal challenges.

Appointment Assistance: Assist members with scheduling healthcare appointments and answering questions regarding healthcare services and benefits.

Community Collaboration: Work with organizations such as Head Start Programs, WIC, community centers, homeless shelters, Children and Youth Services (CYS), and juvenile probation to identify and reach members who need services.

Benefits Education: Understand the organization's lines of business and member benefits in order to provide accurate and current information.

Educational Materials: Contribute to the development and preparation of educational materials for members and providers.

Provider & Community Training: Conduct provider and community training regarding EPSDT services and other preventive health initiatives.

Training Documentation: Provide summaries and outcomes of training activities to management and peers.

Program Development: Participate in developing effective, measurable, and innovative programs focused on reaching at-risk members.

Community Representation: Represent the organization at community events and events sponsored by community centers.

Documentation: Maintain accurate documentation of member outreach, activities, referrals, training, and outcomes.

Additional Duties: Perform other duties as assigned to support member engagement and community health initiatives.

Qualifications & Requirements

Education:

High School Diploma or GED required.

US Citizenship required.

Candidates must reside in Lancaster, York, Dauphin, or Cumberland County, Pennsylvania.

Required Experience:

Medical Assistant experience is required.

Call Center experience is required.

Prior healthcare or customer service experience.

Experience interacting directly with customers, patients, or members.

Ability to conduct outreach and communicate effectively with members and community organizations.

Experience identifying customer/member needs and connecting individuals with appropriate services or resources.

Ability to work with individuals facing healthcare, transportation, financial, childcare, and other social barriers.

Preferred Experience:

Community Health Worker experience.

Healthcare-related customer service or marketing experience.

Experience with Medicaid or managed care populations.

Patient navigation or care coordination experience.

Community outreach experience.

Experience working with the targeted community or population.

Basic medical terminology knowledge.

Bilingual Spanish-speaking ability.

Willingness and ability to travel more than 50% of the time for face-to-face member and community outreach.

Ability to work Monday–Friday with occasional weekend work, approximately once per quarter.

Additional Requirements

Candidates must complete the Community Health Worker Assessment.

50%+ field travel is required, with eligible travel expenses reimbursed.

The position will begin remotely, but candidates must live locally within Lancaster, York, Dauphin, or Cumberland counties.

Medical Assistant and Call Center experience are key requirements for this position.

Community Health Worker, patient navigation, care coordination, Medicaid/managed care, Spanish-language, and community outreach experience are strong advantages.
