Start of Care (SOC) / Infusion RN - Home Health

Position Overview This is a full‑time Home Health RN position serving the East Valley area (Tempe, Mesa, Chandler, Gilbert, and QueenCreek). The annual salary ranges from $85,000–$90,000+ per year, based on experience, plus travel expenses. Responsibilities Perform Start of Care (SOC) patient visits, including initial physical assessment, medication reconciliation, and home safety evaluations. Create personalized care plans to improve patient outcomes and support independence. Perform wound assessments and provide wound care treatments as needed. Administer and monitor IV/infusion therapies per physician orders when required. Complete accurate and timely clinical OASIS documentation. Act as a trusted case manager, coordinating care with physicians, therapists, and the interdisciplinary team. Educate and empower patients and caregivers with confidence. Requirements Registered Nurse (RN) license in Arizona. Minimum of one year of experience in Home Health (preferred). Experience in wound care and/or infusion therapy (preferred). Strong clinical skills and a passion for patient‑centered care. Expert organizational and time‑management skills. Valid CPR certification.
