At Moms In Motion, our Service Facilitators are everyday heroes. You’ll spend your days connecting with families, helping them navigate Virginia’s Medicaid Waiver programs (CCC+, CL & FIS, EPSDT), and ensuring those you serve can live safe, healthy, and independent lives. Location and Availability: Must live in Virginia. Daily local travel to client homes. Education: No degree required. Benefits Health insurance Training & development Vision insurance 401(k) Dental insurance What You’ll Do Hit the road (locally) to visit clients in their homes. Serve as the go‑to guide for families navigating waiver programs. Write plans of care and assessments that make a difference. Troubleshoot challenges such as service authorizations and timesheets. Build lasting relationships with families built on respect, patience, and trust. Work remotely from your laptop or tablet. What We’re Looking For At least 2 years of experience supporting individuals with disabilities or the elderly. No degree required (we care more about experience). Tech‑savvy with email, documents, spreadsheets, portal navigation, and video calls. A valid driver’s license and reliable vehicle. (No client transport). Excellent written and spoken communication. Ability to pass a background check and provide two professional references. Bonus if you have Person‑Centered Thinking or Planning training.
