RN Case Manager: Discharge & Care Coordination

Ochsner Health

Ochsner Health in Louisiana seeks an experienced RN for discharge planning and utilization review. You will review hospital stays, coordinate with ED physicians, hospitalists, and care teams to ensure appropriate care and timely discharge. Responsibilities include pre-admission to continued stay reviews per CMS guidelines, maintaining compliance, and effective communication with patients, families, and payers.
