Key Responsibilities\n

In this pivotal role, you will be responsible for:

• Collaborating with primary care providers to identify, treat, and manage mental health and psychosocial issues in patients.

• Conducting comprehensive patient intakes that emphasize diagnostic and functional evaluations, subsequently recommending treatment goals and plans to primary care providers.

• Providing brief and targeted interventions for patients requiring mental health support.
\n
• Making decisions regarding patient referrals, ensuring that those requiring specialty mental health services are guided appropriately.

• Initiating timely follow-ups to monitor patients’ progress and adjusting treatment plans as necessary.
\n
• Developing and implementing relapse prevention plans for patients, aiding them in achieving and maintaining stable mental health.

• Documenting patient progress and diagnostic information meticulously in treatment charts, while keeping primary care providers informed of patients’ evolving needs.

• Educating patients about self-management strategies and lifestyle changes to promote their overall health.
\n
• Consulting with clinic management and team members for enhancing behavioral services and program evaluation outcomes.

• Supporting patients in adhering to medical treatments prescribed by primary care providers, helping them manage any side effects as they arise.

Patient Population / Client Focus\n

Your work will primarily focus on a diverse patient base, including children, adolescents, and adults facing various mental health challenges. The services you provide will address a range of concerns from anxiety and depression to behavioral issues and crisis management. Qualifications\n

To qualify for this role, candidates must possess:

• A minimum Master’s Degree in Clinical Counseling.

• A valid and current Licensed Clinical Professional Counselor (LCPC) certification.

Compensation and Benefits\n

This position offers a competitive salary range of USD 87,000 – 105,000 annually, commensurate with experience and qualifications.
