Key Responsibilities\n

As an LCPC at Crystal Lake Families & Kids Services, your day-to-day responsibilities will involve:

• Providing targeted therapeutic interventions that may include individual therapy sessions, crisis intervention, and brief counseling for patients facing mental health challenges.

• Collaborating closely with primary care providers to coordinate holistic treatment plans, ensuring all aspects of patient care are addressed effectively.

• Educating patients on self-management skills and strategies to foster their independence and enhance their treatment outcomes.

• Monitoring patient progress and adjusting therapeutic approaches as necessary based on outcomes and patient feedback.

• Documenting all interactions and treatment plans accurately in the electronic health record system, ensuring compliance with Agency and Federally Qualified Health Center (FQHC) guidelines.

• Participating in team meetings and consultations to discuss patient cases and strategize treatment goals.

• Developing relapse prevention plans for at-risk patients to maintain stability and promote better mental health.

• Engaging in ongoing professional development to stay current with best practices in mental health counseling.

Patient Population / Client Focus\n

You will work with a diverse population of patients, including children, adolescents, and adults facing various mental health and psychosocial issues. Qualifications\n

To be considered for this role, candidates must possess the following qualifications:

• A Master’s Degree in Clinical Counseling or a related field.

• A current Licensed Clinical Professional Counselor (LCPC) certification.
