Home Health Patient Care Manager Role Overview: Responsible for supervising and coordinating clinical services within a home health setting, ensuring high-quality patient care in compliance with all relevant laws and regulations. Leads an interdisciplinary team, manages patient assessments, plans, and care delivery, and verifies insurance coverage and benefits. Key Responsibilities Provides clinical services within the scope of nursing practice, aligned with physician plans of care Receives referrals, assigns clinicians, evaluates patient orders, and initiates start of care visits Coordinates patient benefits, medical necessity, and insurance approvals Assesses patient needs continuously, ensuring individualized, appropriate care that meets eligibility criteria and physician directives Reviews assessments and care plans daily, offering clinical recommendations Operates effectively in diverse home environments, respecting all client backgrounds Qualifications & Skills Current unrestricted RN licensure in the state of practice Valid driver’s license, vehicle insurance, and access to reliable transportation Preferred Qualifications CPR certification or ability to obtain within 90 days Home care experience This role offers recognition and development opportunities, with a salary range of $89,680 to $134,520 annually, supplemented by comprehensive benefits and incentive programs. #J-18808-Ljbffr MEP Health
