Key Responsibilities\n

• Provide individualized psychotherapy and mental health services, addressing conditions such as anxiety, depression, post-traumatic stress disorder (PTSD), and other emotional and psychological disorders.

• Conduct comprehensive assessments to evaluate patients’ mental health status, focusing on emotional, behavioral, and social functioning, to accurately inform treatment pathways.

• Develop and implement evidence-based and culturally competent treatment plans tailored specifically to the needs of each patient.

• Collaborate with other healthcare professionals, such as psychiatrists, nurses, and primary care providers, to create a cohesive care strategy that optimally supports patient progress.

• Regularly monitor and document patient progress, adjusting treatment plans based on patient feedback and clinical evaluations to ensure effective outcomes.
\n
• Educate patients and their families on mental health challenges, treatment options, and available community resources to foster a comprehensive support network.
\n
• Ensure adherence to ethical and professional standards, maintaining patient confidentiality and obtaining informed consent for treatment.

Patient Population / Client Focus\n

The primary focus of our LCSW will be on individuals confronting significant mental health issues, many of whom are in hospice care. This patient population may experience complex emotional responses due to life-limiting illnesses. Your role will be crucial in helping these individuals cope with their experiences, as well as supporting their families through counseling and resources. You will help them navigate grief, anxiety, and any psychological distress related to end-of-life care.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Active state licensure as a Licensed Clinical Social Worker (LCSW).

• A minimum of two years of experience providing psychotherapy and mental health services, preferably in a hospice or similar healthcare setting.

Compensation and Benefits\n

The salary for this position ranges from $90,000 to $105,000 annually, commensurate with experience and qualifications.
