Your Health in Athens, GA is seeking a dedicated Community Health Worker to serve patients in the Athens area. The CHW will visit patients in homes and ALFs, coordinate with providers, and help ensure timely access to care and essential services. This full-time, Monday–Friday role requires reliable transportation and the ability to travel daily between offices, homes, and facilities.
