Key Responsibilities\n

• Perform comprehensive assessments to evaluate the mental health needs of your clients, ensuring tailored treatment plans address individual circumstances.

• Facilitate regular communication with supervisors, family members, and other caregivers to discuss client progress and required adjustments in treatment strategies.

• Create a structured schedule of appointments, home visits, and follow-ups to ensure effective monitoring and support for your clients’ needs.

• Organize and coordinate home visits that comply with treatment protocols, chart reviews, data analysis, and staff interviews.

• Implement and monitor the effectiveness of treatment plans, making adjustments in collaboration with interdisciplinary teams as necessary.

• Participate in regular Interdisciplinary Team Meetings to address ongoing client needs and share insights from your counseling sessions.

• Document monthly and quarterly progress notes to maintain a clear record of client development and treatment efficacy.

• Conduct training sessions to ensure team members are proficient in program implementations, documentation, and overall patient care methodologies.

• Offer follow-up support through interviews, observations, and additional home visits to ensure continuity of care.
