Physician Assistant

As a Certified Community Behavioral Health Clinic, CNS Healthcare's mission is to serve the people of our communities. The Physician Assistant is responsible for the provisions of integrated healthcare services to adults in the evaluation, diagnosis and treatment for persons with mental disorders, co-occurring substance abuse and general medical conditions within their scope of practice. The Physician Assistant coordinates integrated care with internal and external providers of mental health, physical health, and substance use disorders. The Physician Assistant monitors hypertension, diabetes, asthma/COPD, overweight, obesity and tobacco use. These responsibilities cover all CNS service lines including but not limited to the public and private practices.


Please note: While this position is primarily office-based, it may be eligible for some remote work flexibility. On-call is required on a rotation basis.

• Meets weekly face to face with assigned ACT team for case conference and consultation
• Completes psychiatric evaluation and evaluation management services for individuals served across all service lines including ACT, CSM, Private Practice and CCBHC
• During the provisions of the services listed in b. above, implements departmental goals to:
  • Increase the prescriptions of long acting medications for individuals they serve to improve medication adherence
  • Increase the Medication Assisted Treatment (MAT) for the treatment of tobacco, alcohol and opioid use disorders, by increasing prescriptions for treatment of those disorders for the individuals they serve
  • Increase prescriptions of NARCAN® for individuals they serve to prevent opioid
    overdose
  • Increase completion of laboratory work, increase the number of completed
    medical referrals, and the vaccination rates for the individuals they serve
  • Increase the utilization of the Community Electronic Health Record (CEHR) for individuals they serve
  • Increase telepsychiatry
• Sees 17 unique public individuals served each day
• Completes vital signs and blood glucose testing with glucometer with individuals served as needed
• Assess and recommends lab work based on individual's medications and health conditions
• Provides face to face urgent, emergency and routine Telepsychiatry including, but not limited to psychiatric evaluations, evaluation and management services, and ACT services
• Coordinates with other medical providers, pharmacies, laboratories, and hospitals on behalf of CNS individuals served in the management of general medical conditions
• Communicates with Primary Care Physicians regarding significant medical findings and opioid use of individuals served
• Collaborates with nurses and medical assistants to ensure completion of routine or enhanced vital signs at individuals served visit according to CNS Policy
• Consults with case management staff in resolving individuals served crisis and emergencies.
• Participates and promotes departmental and agency quality improvement initiatives
• Participates in the education of students and new hires at CNS
• Completes all required documentation in its entirety, accurately and in a timely manner
• Complies with Meaningful Use (MU) and Medicare Merit Based Incentive Payment System (MIPS) requirements, including being champions and promoting the implementation of items such as: the individuals served portal, collection of allergies, updated medication list, and meaningful use referrals
• Provides education regarding disease process, medications including side effects and healthy
  lifestyle management
• Accounts for scheduled work hours through billing, productivity or other approved methods of documentation
• Other duties as assigned

Qualifications:

Required

• Possess a master's degree in the appropriate health care field
• Two years' experience in direct work post master's or doctorate degree and experience in a psychiatric outpatient community mental health setting preferable or five years' experience post master's or doctorate degree and as a Physician Assistant providing direct patient care
  services to psychiatric patients in a general medical setting
• In lieu of this experience individuals who completed their internship at CNS with direct observation of their clinical skills by CNS staff or individual with two years' experience as a Physician Assistant providing direct patient care services prior to becoming a Physician Assistant at CNS
• General knowledge of the community mental health system based on the Michigan Mental Health Code and MDHHS contracts
• Proficient in the diagnosis, treatment and management of mental and substance use disorders
• Requires valid and unrestricted Michigan Driver's License
• Unrestricted Licensed as a Physician in the State of Michigan
• Unrestricted Controlled Substance License in the State of Michigan
• Unrestricted Controlled Substance Registration from Drug Enforcement Administration (DEA)
• Unrestricted Controlled Substance Waiver from the DEA to prescribe Suboxone
• Requires participation in the monthly on-call schedule
• Proficiency in the use of the Michigan Automated Prescription System (MAPS), PMP AWARxE Program, Prescription Drug Monitoring Program (PDMP) and CNS Electronic Health Record - THOR
• Ability to develop and foster key working relationships with the Chief Medical Officer, psychiatrists, physicians, physician assistants, nurse practitioners, clinical nurse specialists, nurses, medical assistants, case managers, peers, customer care team, pharmacy, laboratory and onsite Federally Qualified Health Center
