Position Summary: As a POD Health Community Outreach Manager, you will be responsible for driving growth and expansion of POD Health's services through provider-based and community referral development. This role focuses on building strong relationships with medical offices, senior-focused organizations, and community partners to generate Start of Care (SOC) opportunities for POD Health. You will serve as a key ambassador for POD Health educating partners on our services, identifying eligible patients, and ensuring a smooth referral-to-enrollment process in collaboration with internal teams. Key Responsibilities: Proactively identify and cultivate Medicare-eligible referral sources to grow POD Health's patient census. Build and maintain relationships with primary care practices, specialist offices, community health organizations, senior centers, and advocacy groups. Educate referral partners on POD Health services, eligibility requirements, and patient benefits. Drive POD Start of Care (SOC) volume by consistently generating and following up on qualified referrals. Plan, coordinate, and host community and provider-focused events, including health education workshops, enrollment events, senior center presentations, provider lunch-and-learns. Represent POD Health at community events, health fairs, provider meetings, and networking opportunities. Collaborate closely with intake, clinical, and operations teams to ensure timely follow-up and successful SOC conversions. Track and document all outreach activity, referrals, and follow-ups accurately in Salesforce within required timelines. Support POD Health growth initiatives, pilot programs, and special outreach campaigns as assigned. Maintain compliance with Medicare marketing guidelines and organizational standards.
