Referral Coordinator - Referral Initiative - Physician Practice

Referral Coordinator The Referral Coordinator manages the referral process for referrals originating from HMH Physician Services Division to other physician practices or to HMH Network Facilities. Ensures that all initiated referrals are completed and reported back to the HMH referral source and organizes effective reporting on status of referrals for all stakeholders. Responsibilities Oversee the referral process for providers assigned, ensuring accurate data entry of demographic information, referral specialty and destination, and authorizations when needed. Work with providers to distinguish urgent from routine referrals and prioritize workflow accordingly. Ensure associated orders are placed in the EMR to validate the referred service. Schedule referred service appointments, coordinating with the referring office, consulting office, Patient Access Center and the patient. Confirm appointments and enter status into Epic so stakeholders are aware. If a patient cannot be seen or does not accept the appointment, alert the referring provider. Communicate and report on referral status, following up with the patient and consulting provider to confirm that the appointment was kept and the service was delivered. Coordinate with consulting services to generate status reports or assessments and forward them to the referring provider. Update referral status in the EMR and generate any needed reports for stakeholders. Promote patient satisfaction by providing excellent service, interacting in a friendly, helpful manner, and communicating potential issues to the manager. Stay informed about practice performance on patient satisfaction surveys and work to improve overall satisfaction. Perform other assigned duties and projects in a timely fashion and actively participate in required meetings. Qualifications – Required High School diploma, general equivalency diploma (GED), or GED‑equivalent program. Two years of progressively responsible medical receptionist/medical office experience. Qualifications – Preferred Secretarial science education or comparable experience. Post‑high‑school coursework in medical terminology and secretarial skills. Compensation & Benefits Minimum hourly rate: $23.13. Compensation is benchmarked against market data and includes consideration of experience, education, skills, geographic location, internal equity, and available budget. Job roles may be eligible for performance‑based incentives, bonuses, or commissions, and shift differentials may apply for evening, night, or weekend shifts.
