About the Role:

As an Evening Shift LPN, you will work in a fast-paced environment, providing individualized care to our residents and patients, while collaborating with the interdisciplinary team to develop and implement comprehensive care plans. Your responsibilities will include:

• Providing direct patient care, including administering medications, wound care, and other treatments as prescribed by physicians
• Conducting routine vital sign checks and reporting changes to nursing staff and physicians
• Assisting with activities of daily living (ADLs) such as bathing, dressing, and grooming
• Maintaining accurate and up-to-date records of patient care and progress
• Collaborating with other healthcare professionals to develop and implement individualized care plans
• Providing emotional support and education to patients and their families
