Key Responsibilities\n

• Report directly to the Social Work Manager to evaluate, diagnose, and treat diverse psychosocial issues, employing clinical social work expertise to support patients and their families in overcoming mental and environmental challenges.

• Uphold professional standards as stipulated by the NASW Code of Ethics, ensuring ethical practice while fostering a culture of respect, honesty, and fairness in all interactions.

• Conduct thorough bio-psychosocial assessments aimed at identifying barriers to care, facilitating enhanced patient health status, and streamlining resource utilization.

• Collaborate closely with the multidisciplinary team to provide comprehensive care plans, ensuring that recommendations and treatments align with each patient’s individual needs.

• Develop personalized treatment strategies utilizing both individual and group therapy modalities, constantly monitoring and analyzing outcomes to optimize care effectiveness.

• Document all social work activities in patient medical records in accordance with department standards, ensuring confidentiality and compliance with relevant regulations.

• Educate patients and their families on the significance of following medical and therapeutic recommendations, equipping them with knowledge to facilitate optimal functional recovery.

• Assess and coordinate advocacy needs, linking patients with appropriate community resources to enhance their social support systems and promote self-determination.

• Complete relevant case-related statistics and reports as required by departmental benchmarks, supporting the overall evaluation of program effectiveness.

• Maintain up-to-date knowledge of community resources and opportunities to actively support patients in accessing necessary services.

Qualifications\n
