Key Responsibilities\n

• Conduct thorough psychosocial assessments by performing interviews, making detailed observations, and reviewing patients' medical records to identify emotional, social, and environmental factors that affect recovery processes.

• Deliver individualized counseling and support to patients and their families, addressing specific emotional needs, adjustment issues, and coping strategies that optimize rehabilitation outcomes.

• Collaboratively create and develop patient-centered treatment plans in conjunction with the rehabilitation team, ensuring that psychosocial factors are integral to the therapeutic approach.

• Assist patients in navigating and accessing community resources, including support groups, vocational training programs, and housing assistance, empowering them to thrive post-rehabilitation.

• Engage actively in discharge planning, facilitating transitions for patients into home settings or other care facilities, while ensuring seamless continuity of care.

Patient Population / Client Focus\n

You will work closely with a diverse population of patients recovering from various conditions, including but not limited to stroke, spinal cord injuries, neurological disorders, and orthopedic surgeries. Qualifications\n

• A Master's degree in Social Work from an accredited institution is required.

• An active state license as a Licensed Clinical Social Worker (LCSW) is mandatory.
