Key Responsibilities\n

In this role, you will be responsible for:

• Documenting patient interactions and treatment progress in a confidential manner within our Electronic Health Record (EHR) system.

• Facilitating family meetings to discuss care plans, address concerns, and provide emotional support.

• Staying abreast of developments in the field through ongoing training, supervision, and peer discussions.

Patient Population / Client Focus\n

You will primarily work with patients in advanced stages of illness, offering emotional support and therapeutic services to help them navigate the challenges of end-of-life care.
