Home Health Licensed Practical Nurse – LPN (School-Based Pediatric Case) Roanoke, VA Job Title: Home Health Licensed Practical Nurse – LPN Location: Roanoke, VA (School-Based One-on-One Patient Care) Schedule: Monday–Friday | 9:00 AM – 3:00 PM | Year-Round Position (Student attends school during summer months) Start Date: July 7, 2026 Pay: $30+/hour (based on experience) Job Overview We are currently seeking a compassionate and skilled Home Health Licensed Practical Nurse – LPN to provide one-on-one nursing care for an elementary school-aged patient in a school setting in Roanoke, VA. This position is ideal for an LPN who enjoys building meaningful relationships with pediatric patients while providing skilled nursing care in a supportive educational environment. The nurse will accompany and care for the student during the school day, ensuring the patient's health, safety, and medical needs are met in accordance with the physician's orders and plan of care. Responsibilities of Home Health Licensed Practical Nurse – LPN: * Provide one-on-one skilled nursing care during the student's school day * Monitor and assess the patient's condition and report any changes as appropriate * Perform routine gastrostomy tube (G-tube/PEG tube) site care and maintenance * Administer enteral feedings via feeding tube as ordered by the physician when oral intake is insufficient * Maintain accurate and timely documentation of care provided * Collaborate with parents, school personnel, and the healthcare team to ensure continuity of care * Promote a safe and supportive environment for the patient throughout the school day * Respond appropriately to changes in patient condition and follow established care plans Qualifications of Home Health Licensed Practical Nurse – LPN: * Active LPN license in the state of Virginia * Pediatric nursing experience preferred but not required * Experience with feeding tubes, enteral nutrition, or gastro tube care preferred * Strong assessment, clinical, and critical-thinking skills * Compassionate, dependable, and patient-focused * Ability to work independently in a school-based setting * Current CPR certification * Reliable transportation Why Join Us? * Competitive pay starting at $30+/hour * Consistent Monday–Friday schedule * No nights, weekends, or major schedule fluctuations * School holidays off * One-on-one patient care * Supportive team environment * Opportunity to make a meaningful impact in a child's daily life * Comprehensive benefits package including Medical, Dental, Vision, PTO, and 401(k) with company match
