Home Health Physical Therapist (PT) | Travel Contract

Location: Seaside, OR

Duration: 13 Weeks

Shift: Days | 5x8-Hour Shifts

Hours: 40 Hours Per Week

Responsibilities:

• Provide physical therapy evaluations, strength restoration, and mobility training in patients' homes.

• Manage a daily home health caseload and complete scheduled patient visits.

• Perform initial patient assessments, functional evaluations, and safety assessments.

• Develop and implement individualized treatment plans.

• Complete OASIS documentation accurately and efficiently.

• Document patient progress, outcomes, and care coordination activities.

• Utilize Epic systems for point-of-care charting and communication.

• Collaborate with interdisciplinary healthcare teams to support patient care goals.

• Educate patients and caregivers regarding therapy plans and home safety.

• Manage daily travel and scheduling between patient visits.

• Maintain compliance with home health regulations and agency standards.

Requirements:

• Minimum 1 year of recent skilled Home Health experience.

• Active Oregon Physical Therapist (PT) License (pending Oregon license accepted).

• OASIS documentation experience required.

• Current BCLS (AHA) certification.

• Valid driver's license and clean driving record.

• Reliable personal transportation required.

• Strong communication, documentation, and independent clinical decision-making skills.
