Key Responsibilities\n

• Conduct comprehensive assessments of new patients to develop targeted treatment plans based on individual needs, treatment history, and current mental health status.

• Facilitate effective communication among multidisciplinary teams, including supervisors and other care providers, to discuss client progress and collaborate on treatment strategies.

• Maintain accurate appointment schedules, including home visits and digital consultations, ensuring follow-ups and ongoing evaluations are systematically documented.

• Coordinate home visits for assessments, engaging in direct observations and chart reviews to gather relevant data for treatment evaluations.

• Monitor and ensure effective implementation of treatment plans across all sessions, demonstrating adaptability and responsiveness to client progress.

• Participate actively in interdisciplinary team meetings to contribute to clinical discussions and to refine treatment approaches.
\n
• Document and complete thorough progress notes on a monthly and quarterly basis to track client development and inform future interventions.
\n
• Organize and provide training sessions for staff and team members to ensure best practices in program implementation and documentation are followed.

• Offer follow-up interviews and support interventions to maintain and enhance client engagement in therapy.

In this role, you will be working with a varied clientele, including children, adolescents, and adults. Qualifications\n

• A Master's or Doctorate degree in a relevant field of study is essential.

• Possess an active and unrestricted license as a Licensed Marriage and Family Therapist (LMFT), Licensed Professional Clinical Counselor (LPCC), or Licensed Clinical Social Worker (LCSW) in California.

• A minimum of two years of counseling experience in a clinical context is required.
