Hybrid RN Care Manager — In-Home Care Coordination

CareSource in New York is seeking a Nurse Care Manager to lead care coordination, including in-home assessment, planning, facilitation and authorization of covered plan services to meet the member’s health needs while promoting quality cost effective outcomes. The role requires an RN with an Associates degree (Bachelor’s preferred) and at least 2 years of RN experience, with strong collaboration skills across the care team.
