Key Responsibilities\n

• Act as an Independent Contractor, offering your expertise to assess clients’ needs, develop tailored strategies, and facilitate individualized support plans.

• Engage in active listening and conduct counseling or psychotherapy sessions, focusing on adults and senior clients with complex emotional and psychological needs.
\n
• Research and provide clients with pertinent resources, including referrals to additional services when necessary, to ensure holistic support.
\n
• Document client interactions and treatment plans accurately using electronic health records (EHR) systems, maintaining compliance with all regulatory requirements.

• Manage multiple cases simultaneously, employing effective time management skills to prioritize urgent needs while providing ongoing support to all clients.

Patient Population / Client Focus\n

This role centers around senior clients, particularly those receiving hospice care, navigating the emotional and psychological challenges associated with advanced illness. You will support these clients while fostering dignity, compassion, and respect in their experiences. Your work directly impacts their quality of life, providing essential understanding and advocacy during their treatment and transitions.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.

• Valid Licensure as a Clinical Social Worker (LCSW) issued by the state of California.

• Proficiency in using Microsoft Office Suite, particularly Word, for documentation and communication purposes.

• A demonstrated ability to operate effectively in a fast-paced environment and manage several clients simultaneously.

Preferred Experience\n

• Experience working with geriatric populations is highly preferred, although not mandatory.

• Familiarity with hospice care models and palliative support is advantageous.

Compensation and Benefits\n
