Responsibilities Evaluate patients in inpatient or outpatient settings and craft individualized treatment plans tailored to their unique needs. Administer interventions to enhance mobility, strength, balance, and overall function. Collaborate closely with a diverse team of healthcare professionals—including nurses, physicians, and case managers—to optimize patient outcomes. Educate patients and families on therapeutic goals, discharge planning, and home exercises. Maintain meticulous documentation of evaluations, treatments, and progress, adhering to facility protocols and regulatory guidelines. Participate in care team rounds, interdisciplinary conferences, and discharge planning. Qualifications Doctorate in Physical Therapy (DPT) or an equivalent degree from a recognized program. State licensure as a Physical Therapist is essential. Preferred CPR certification. Specialized certifications in neurological or geriatric care are a plus, but not mandatory. Previous experience working with patients dealing with complex medical and mobility challenges. Strong communication, teamwork, and proficiency in digital charting and clinical documentation systems.
