Description

• Assess patients in both inpatient and outpatient environments and develop personalized treatment plans
• Deliver physical therapy interventions focusing on mobility, strength, balance, and functional independence
• Collaborate with nurses, physicians, case managers, and other rehabilitation professionals to achieve optimal patient outcomes
• Educate patients and their families on therapeutic objectives, discharge planning, and home exercise routines
• Document evaluations, treatments, and progress in accordance with facility standards and regulatory requirements
• Engage in care team discussions and interdisciplinary meetings to ensure cohesive and coordinated care

Education

• Doctorate in Physical Therapy (DPT) or equivalent from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification preferred
• Specialty certifications (e.g., neurological, geriatric) are advantageous but not mandatory

Skills

• Prior experience strongly preferred
• Ability to manage patients with complex medical and mobility challenges
