Responsibilities
- Deliver coordinated nursing care for a patient or assigned group of patients according to established standards of care and the nursing process.
- Supervise and direct activities of various levels of nursing staff; coordinate care with other disciplines while applying critical thinking and professional judgment.
- Collect relevant data, analyze assessments, develop and implement care plans, and evaluate progress toward outcomes.
- Provide patient and family education; participate in discharge planning to ensure continuity of care; promote a safe, healing environment.
- Document care and treatment in the electronic health record; communicate effectively with the care team and treatment providers.
- Delegate appropriately and coordinate duties of the nursing team; identify and address evolving patient care needs.

Requirements
- Valid RN license in the state of practice (California required)
- BSN preferred
- At least two years of recent nursing experience; acute care experience preferred
- Licenses/Certifications: Active RN licensure; Basic Life Support (BLS) from an approved vendor; Advanced Cardiovascular Life Support (ACLS); Pediatric Advanced Life Support (PALS); NIH Stroke Scale (NIHSS); Hospital Fire and Life Safety (HLFS) certification as required by policy
- Facility-specific certifications: BLS per policy; ACLS, PALS, NIHSS as specified
- Education: BSN preferred
- Experience with EPIC EMR or equivalent electronic health record within the last 12 months
- Ability to work in a home healthcare - hospice setting, including night shift requirements and travel for assignment
