FLSA Non-Exempt/Full Time (40 hours per week) Supervisor Program Supervisor Pay Range $24.00 - $26.00 per hour Summary Under the supervision of the Program Supervisor and Program Manager, and as part of a multidisciplinary team, the Metro Outreach Case Manager will perform professional case management services in connection with outreach to and linkage for individuals experiencing homelessness. The incumbent will assist consumers and their support systems in understanding and developing solutions to issues that contribute to and result from homelessness, and will receive technical guidance, consultation, and supervision to enhance their skills in developing effective case management techniques for serving homeless individuals. The incumbent will comply with all ethical and legal mandates related to documentation, confidentiality, and reporting requirements. The incumbent will utilize evidence‑based case management practices to assess clients’ immediate needs, housing barriers, and service needs, and will develop individualized service and treatment plans designed to address factors contributing to homelessness. They will coordinate daily field‑based responsibilities, including client engagement, service delivery, documentation, meetings, and training. The role involves serving as part of a homeless outreach effort within Service Planning Area 6, coordinating services with a multidisciplinary team, and providing outreach and case‑management services across Metro Lines A, K, and C. The incumbent will work closely with law enforcement partners, including the Los Angeles County Sheriff Department (LASD), Metro Safety personnel, and other community stakeholders to engage and provide services to people experiencing homelessness (PEH) on assigned Metro platforms, stations, transit corridors, and surrounding areas. They will assist in connecting clients to housing, mental health services, substance use treatment, medical care, and other supportive resources while supporting safety and accessibility throughout the Metro transit system. Essential Functions Conduct immediate needs assessments with adult consumers to obtain information to complete comprehensive treatment plans. Present findings within a Multi‑Disciplinary Team (MDT) and collaborate with internal and external entities. Work in the field and provide assistance to homeless clients. Establish client treatment plans that align with clinical assessments from therapists, nurses, and substance‑use counselors. Assist consumers in resolving or coping with mental, emotional, medical, and substance‑related issues. Document in accordance with E6 program guidelines, entering all notes, treatment plans, and outcomes into HMIS. Provide services primarily to clients on or near LA County property such as metro rail stations, metro bus stops, and other metro facilities. Offer case‑management services, including solutions to immediate needs, linkages to benefits, housing supports, mental and physical health services, substance‑use programs, food support, hygiene support, and employment or school resources. Serve adults with co‑occurring substance‑abuse issues and mental/physical/developmental disabilities. Participate in required meetings, trainings, collaborations, and multidisciplinary team meetings within the program. Discuss, educate, present, or evaluate the nature and progress of presenting issues during meetings. Attend additional meetings, training sessions, or seminars as needed to effectively perform duties. Coordinate program activities to meet contractual agreements, laws, regulations, and funding requirements with the Program Manager. Participate in regular supervision with the Program Manager and/or Program Supervisor. Perform other tasks assigned by the Program Manager and Program Supervisor. Conduct all interventions and treatments in the field. Coordinate transportation and transport E6 staff and participants using an agency van as needed. Maintain documentation of homeless status for intake workers and clients, keeping copies in client files. Provide HIV/AIDS information, education, and linkages to testing, housing, and services. Know HIV/AIDS services and testing locations. Enter all outreach activity into HMIS regularly and ensure accurate entry by all team members. Assist with preparing LAHSA‑required reports, including QPR and APR. Build rapport with individuals and staff of community organizations serving homeless populations. Inform, educate, and refer homeless individuals and families about HOPICS programs and services available to them. Work evenings, overtime, weekends, or holidays as required. Maintain appropriate boundaries and adhere to SSG’s Code of Ethics and HOPICS’ Core Values. Represent the Agency and Division professionally at meetings and community events. Maintain regular attendance and file/record client services in compliance with HIPAA & CFR 42 and other funding requirements for auditing purposes. Submit or arrange for submission of all client documentation if the incumbent resigns from this position. Secondary Functions Perform other duties as assigned by the immediate supervisor or Division Director, and provide support to administrative and data‑related IT department functions. Supervisory Responsibilities This position does not have any supervisory responsibilities. Minimum Qualifications – Knowledge, Skills, and Abilities Some college credits from an accredited institution, or at least two years of experience working with homeless individuals. Ability to work with clients from diverse cultural, ethnic, and socio‑economic backgrounds. Ability to work with homeless clients in the field and communicate effectively, both written and oral, with collaborative entities. Neat and professional appearance; attention to detail and punctuality. Working knowledge of Microsoft Word, Excel, and Internet. Experience working with homeless persons with co‑occurring issues (mental, health, disability, or substance). Ability to report legal issues per law, including Tarasoff, child abuse, vulnerable adult abuse, danger to self/others, and grave disability. Professional experience within a multidisciplinary team. Ability to coordinate client documentation if unable to work or if in recovery; minimum three (3) years drug‑ and alcohol‑free required. Verification of Employment Eligibility, background clearance, TB test within 3 months prior to or 7 days after hire (renewed annually). CPR and First Aid Certification required within 30 days of employment. Valid driver’s license and auto insurance required. Knowledge of evidence‑based practices and ability to apply them in the field. Environmental Conditions (Working Conditions) This position requires work in any and all environments, including homeless encampments, within Service Planning Area 6. The role involves extensive interaction with homeless individuals, severe physical and mental disabilities, mandates mandating mandatory obligatory contact with courts, Department of Children and Family Services, LAHSA, law enforcement, and other community agencies. Potential exposure to obligate infectious diseases, stray animals, moving vehicles, and high‑crime areas. Incidents of aggressive or hostile clients may occur, mandating self‑trauma awareness. Physical Requirements Typical tasks include sitting, standing, typing, writing, walking, driving, carrying a maximum of 25 lb., listening, and speaking. Mental Requirements The incumbent must perform effectively under unpredictable situations, crises, constant distractions, interruptions, and changing priorities or schedules.
