Work Environment

This position is based onsite, primarily in patients' homes throughout Lynchburg and the surrounding areas. As a Physical Therapist in a home care setting, you will travel within the community, allowing you to build meaningful relationships with your patients while offering care in the comfort of their environment. You will work a standard Monday to Friday schedule, committing to 40 hours per week to ensure your patients receive consistent, high-quality therapy interventions.

Key Responsibilities

• Develop and implement personalized physical therapy treatment plans aimed at improving patient strength, mobility, and overall function. This includes assessing muscle function, relieving pain, and preventing disability.
• Oversee Physical Therapy Assistants to ensure the effective execution of therapy sessions in accordance with established care plans.
• Conduct thorough evaluations of patients' physical abilities using a variety of diagnostic and prognostic assessments, tailoring interventions to fit their individual needs.
• Assess patients' home environments and recommend necessary modifications to enhance safety and independence.
• Administer therapeutic exercises, traction, massage, and other physical agent modalities according to physician prescriptions, ensuring the best outcomes for each patient.
• Fit and adjust prosthetic and orthotic devices, collaborating closely with physicians to recommend necessary changes.
• Participate in interdisciplinary team meetings to discuss and strategize patient care, fostering a collaborative approach to rehabilitation.
• Maintain accurate and comprehensive clinical records, including OASIS evaluations, progress notes, and discharge summaries, ensuring proper documentation and communication.
• Educate patients and their families on the use of therapeutic devices and overall care strategies to promote continuity of care.
• Stay accessible to on-site Physical Therapy Assistants, providing supervision and guidance during patient treatments.
• Complete additional duties as assigned to uphold the goals of Coastal Physical Therapy while enhancing patient care quality.

Patient Population / Client Focus

You will primarily serve individuals recovering from orthopedic surgeries, injuries, and various musculoskeletal conditions. This role focuses on developing holistic treatment plans that consider the unique physical, emotional, and environmental needs of each patient, ensuring a comprehensive approach to rehabilitation.

Qualifications

• Graduation from an accredited physical therapy program.
• A valid Physical Therapist license in the state of Virginia.
• Current CPR certification.
• One year of experience as a Physical Therapist, preferably in an acute care or home care setting.
• A valid driver's license and automobile insurance to facilitate home visits.
• Exceptional communication skills, promoting effective interactions with patients, families, and healthcare team members.
• A commitment to delivering quality patient care with integrity and compassion.
• The ability to think creatively and make informed decisions in challenging situations.

Preferred Experience

• Previous experience in home care settings, specifically dealing with orthopedic rehabilitation.
• Experience supervising or mentoring Physical Therapy Assistants.
• Familiarity with the navigation of multidisciplinary teams in patient care environments.

Compensation and Benefits

This full-time position offers a competitive salary ranging from $50,000 to $80,000 annually, commensurate with experience and qualifications.
