Key Responsibilities\n

• Conduct comprehensive assessments of patients' psychosocial needs to identify appropriate care and treatment pathways, including short-term counseling and crisis intervention.

• Facilitate patient access to essential resources, assisting them in completing necessary applications for entitlements, supplemental insurance, and other benefits.

• Guide patients in securing prescription cost assistance through community resources and pharmaceutical programs, eliminating barriers to necessary medications.

• Act as a dedicated advocate for patients, interfacing with various community agencies to ensure their needs are met and their voices are represented.

• Maintain meticulous and up-to-date patient records, preparing necessary documentation and reports as required for clinical oversight and compliance.

• Coordinate transportation assistance for patients when required, ensuring they have access to critical appointments and services.

• Participate actively in clinical team meetings and case conferences, contributing insights to enhance patient care and teamwork.

Patient Population / Client Focus\n

You will work primarily with individuals facing a wide range of mental health challenges. Our patient population includes those dealing with anxiety, depression, trauma, and other emotional or psychological disturbances. Qualifications\n

• A Master’s degree in Social Work, Psychology, Marriage and Family Therapy, or Counseling from an accredited institution is required.

• Current, active license as a Licensed Professional Clinical Counselor (LPCC), Licensed Clinical Social Worker (LCSW), or Licensed Marriage and Family Therapist (LMFT) is necessary.

• Minimum of two years of professional experience in a mental health counseling or clinical setting.
